Care at home and support is crucial to ensuring that people get the help they need to live independently and stay supported at home for as long as they can, reducing the risk of prolonged hospital stays.

A national post and project has been funded to look at how these services can be transformed to meet future demand utilising technology and within the environment of integration.

This national project is funded by Social Work Scotland, Scottish government, Coalition of Care Providers Scotland, Scottish Care and Macmillan Care Support. It has been developed in partnership with COSLA.

The final report will be published in the Autumn of 2018.
